Physical Examination

Students registering for athletics must have a current physical examination on file with the Athletic Trainer before the student is authorized to participate in conditioning/weightlifting or practice/tryouts. Each physical should be completed fully, with every signature signed and insurance included on the last page.

Physicals are valid from May 1st of the current year through June 30th succeeding year.
